Telehealth Informed Consent & Disclosure

THE TRTBOSS PLATFORM IS NOT DESIGNED FOR EMERGENCY OR URGENT MEDICAL CARE. IF YOU BELIEVE YOU ARE EXPERIENCING A MEDICAL EMERGENCY, CALL 911 OR GO TO THE NEAREST EMERGENCY DEPARTMENT IMMEDIATELY.
1. Purpose of This Informed Consent
This Telehealth Informed Consent & Disclosure (“Consent”) explains:
How telehealth services are provided through the TRTBOSS platform
The benefits, risks, and limitations of telehealth
Your rights and responsibilities as a patient
The roles of the platform, clinicians, and other third parties
This Consent is provided to satisfy state telehealth informed consent requirements and must be affirmatively accepted before you receive telehealth services.
This Consent supplements, but does not replace:
Treatment-specific informed consents
Clinical discussions with your provider
Any additional disclosures required by law
2. Who Provides Your Care
Medical services accessed through the TRTBOSS platform are provided by independent, licensed healthcare professionals (“Clinicians”) who practice within the scope of their licensure and exercise independent medical judgment.
VALKORA LLC, doing business as TRTBOSS (“TRTBOSS”), is a non-clinical technology company and management services organization that:
Provides technology, administrative, and support services
Does not practice medicine
Does not diagnose, treat, or prescribe
Does not control or influence clinical decision-making
All medical decisions are made solely by your treating Clinician.
3. What Is Telehealth
Telehealth is the delivery of healthcare services using electronic communications when the patient and provider are not in the same physical location.
Telehealth services may include:
Video conferencing
Audio-only (telephone) encounters
Secure messaging
Asynchronous questionnaires or forms
Transmission of images or health information
You acknowledge that during telehealth visits, your provider is not physically present with you.
4. Telehealth Consent and Affirmative Agreement
By proceeding, you affirmatively consent to receive healthcare services via telehealth and acknowledge that:
Telehealth involves the use of electronic communications and information technology
Telehealth may have different risks and benefits than in-person care
Telehealth may not be appropriate for all conditions or situations
You understand that you may withdraw consent for telehealth services at any time by discontinuing use of the Platform or notifying your care team, subject to applicable law.
5. Benefits, Alternatives, and Limitations
Potential Benefits
Telehealth may offer:
Increased convenience
Improved access to care
Reduced travel and wait times
Alternatives
You understand that alternatives to telehealth include:
In-person medical care
Seeking care from another provider
Choosing not to receive treatment
Limitations
You acknowledge that:
A physical examination may not be possible
Diagnostic accuracy may be limited by the information available
Delays, interruptions, or technical failures may occur
Telehealth may result in delayed diagnosis or treatment in some cases
No specific medical outcome or improvement is guaranteed.
6. Risks of Telehealth
Potential risks of telehealth include, but are not limited to:
Technical failures, delays, or disconnections
Poor audio or video quality
Incomplete or inaccurate information affecting clinical decisions
Unauthorized access despite reasonable safeguards
Limitations inherent in remote evaluation
If technical issues compromise the quality or safety of care, your Clinician may:
Discontinue the encounter
Reschedule the visit
Recommend in-person evaluation or alternative care
7. Privacy and Security
Telehealth services involve electronic transmission of health information.
To protect confidentiality:
You are encouraged to participate from a private location
Providers conduct visits from private settings
You are responsible for securing your devices, networks, and surroundings
Health information may be used and disclosed for treatment, payment, and healthcare operations as permitted by law and described in the applicable Notice of Privacy Practices.
8. Access to Medical Records
Telehealth providers may rely on:
Information you provide
Records you authorize access to
Providers may not have access to your complete medical history, which may affect evaluation or diagnosis. You consent to this limitation.
9. When Telehealth Is Not Appropriate
A Clinician may determine that telehealth is not clinically appropriate based on your condition, symptoms, or available information.
In such cases:
Your request may not be submitted for clinical review
You may be advised to seek in-person, urgent, or emergency care
10. Prescriptions, Pharmacies, and Laboratories
Participation in telehealth services does not guarantee that:
A prescription will be issued
A particular medication will be prescribed
Prescribing decisions are made solely by Clinicians in accordance with:
Professional judgment
Applicable state and federal law
Certain medications, including controlled substances, may be subject to additional legal requirements or may not be available via telehealth.
You may request to use a pharmacy or laboratory of your choice, subject to legal and practical constraints.
11. Asynchronous Communication and Response Times
Some communications through the Platform may be asynchronous and are not monitored continuously.
You acknowledge that:
Response times are not guaranteed
The Platform must not be used for urgent or emergency conditions
12. Emergency Care
Telehealth services accessed through the Platform do not address medical emergencies.
If you believe you are experiencing an emergency:
Call 911, or
Go to the nearest emergency department immediately
13. Patient Responsibilities
You agree to:
Provide accurate, truthful, and complete information
Update your care team regarding changes in your health
Follow Clinician instructions
Seek in-person care when advised
Incomplete or inaccurate information may affect clinical decisions.
14. Minors
The Platform is intended for adults only.
Telehealth services are not provided to minors.
15. Recording of Telehealth Visits
Telehealth visits are not recorded unless:
You are notified in advance, and
Your consent is obtained, or
Recording is otherwise permitted by law
Patients may not record visits without provider permission where required by law.
16. Specialty Services and Additional Consents
Certain services—such as testosterone replacement therapy (TRT) or other specialty treatments—require additional, treatment-specific informed consent.
Telehealth consent alone is insufficient to receive such services.
17. Voluntary Consent and Acknowledgment
By proceeding, you acknowledge that:
You have read and understand this Consent
You had the opportunity to ask questions
You understand the risks, benefits, and limitations of telehealth
You voluntarily consent to receive telehealth services as determined appropriate by your Clinician
18. Changes to This Consent
This Consent may be updated from time to time.
The version in effect at the time of your encounter governs.
